Janet Duni, RN, BSN, MPA, CCM

Janet is a highly skilled nurse executive dedicated to helping patients navigate the healthcare system through integrated, patient-centered solutions. She specializes in building collaborative care teams and medical “neighborhoods” that support value-based care and its goal of improved health.

Janet began her clinical career in critical care, working in ICU and emergency room settings before transitioning to hospital case management. An early adopter of the patient-centered medical home model, she was one of eight nurses selected to pilot this innovative approach to community-based care in New Jersey.

​

In 2013 she joined Vanguard Medical Group as Director of Care Coordination, where she developed and implemented a robust and flexible care coordination program which has directly supported population health efficacy and decreased the cost of care. She helped grow client sites from one in 2011 to eight in 2019, serving 80,000 patients.

​

During her time with Vanova Health Solutions, Janet transitioned an innovative, cost-effective care coordination program from a single primary care practice to one that serves multiple practices within a clinically integrated network. She supervises a team of registered nurses, social workers, behavioral health specialists and population health specialists. As one example of her impact, her team makes approximately 125 behavioral health referrals per month while providing wraparound support, from coordinating equipment and resolving insurance issues to ensuring seamless hospital-to-home transitions.

Janet holds a Master's degree in Health Policy and Administration from Columbia University and earned her B.S. in Nursing from Rutgers University. Her unique blend of critical care experience, policy expertise, and leadership bridges the gap between clinical complexity and practical, patient-centered solutions.
